The secure Patient Portal can be used to:
      » request and manage appointments
        » send secure messages to your provider or the support staff
          » access your medical records including lab results and visit summaries
            » share documents with your provider.
Scheduling Appointments:
Please make all appointment requests via the calendar or from your Personal Health Record.
All request are electronically sent to be approved. After review, a
notification will be sent to you via your personal email address that is on file.
If you require an urgent appointment, please include the word "URGENT" along with
the reason you want to be seen.
To Make A New Appointment Request:
        New patients: Access theCalendar
                         
Established patients: Visit the
Patient Portal
If you have forgotten your login information please visit the Patient Portal and request a
password reset there.
